Traditionally the humble internet form has caused web-designers to stumble, trip and fall - and it has been one of the least-favourite aspects of html design as it can oh-so-often go wrong. Here's a list - however - of forms which work beautifully from designers who mostly have created their forms on codepen.

Each form is different from the next, although they all share some fundermental principles in their design. They are all 'responsive' and should render perfectly well on all screen sizes. You can test this by re-sizing the browser window - or simple by navigating to this page on your device of choice. I have provided the name of the author and a download link so that you can download and experiment with the forms yourself.

Clean Contact Form

a clean looking contact form with an image in the form header
By Nick Haskell

The main feature of this form is that it uses a background image for the top of the form, allowing you to make it seem more visually appealing, and in many ways more professional. The code is written clearly and is easy to understand as well as modify as, indeed, I have done.

Responsive Form

a responsive contact form with a blured background image
By Lisa Wagner

This mobile optimized form uses boxed layout with a special feature ‘dropdownds’. The dropdowns can be specified to help you better categorize the emails that you are receiving, and in turn either allocate them priority or just have a better understanding of the kind of support people are seeking.

Minimalistic Form

a minimalistic contact form with clean and simple lines
By Matheus Marsiglio

This form from Matheus Marsiglio is truly a definition of minimal design. There’s nothing but a single box that will collect all the contact details and transfer them to the actual dynamic script that takes care of emails, and the only fancy thing is the overlay effect when you hover over different parts of the required details.

Plain Contact Form

a plain, uncluttered contact form
By Tim Quincey

Tim Quincey sees the design world as a simple canvas (or landscape) that offers unlimited possibilities, but the real magic happens in the process of filling out forms and data and letting the user experience the unfolding of a successful submission. As well as the rosy pink gradient background, Krishnan has taken the time to create a javascript text animation to trigger when you tap inside an input box. Very friendly!

Drop-Down Form

an appointment form.
By Vincent Durand

Vincent Durand has created here a beautiful drop-down contact form with a fun 'sliding' animation on input:hover. The whole thing works very well on smaller screens and other devices (at least, all that I've tested it on so far) and the only criticism I have is that it doesn't appear to be particularly accessible. This is where I can affect some change.
